Physical Therapy Evaluation

This is a 45 minute session with a licensed physical therapist, providing a comprehensive evaluation of your spine. We ask that you bring all relevant information, including any recent imaging studies or evaluations that have been performed.

This is a 45 minute one-on-one physical therapy session with a licensed physical therapist. During this individualized experience, we will provide you with an individualized program to address your specific needs.

Prior to your first visit we ask you to complete several forms. For your convenience we have made these forms available for you online.

Our online intake form can be completed online through our secure server. If you feel uncomfortable doing this, you may also download this as a PDF file and bring it to the clinic during your first visit.
